SERVICE PROVIDER

Job Description

 

PURPOSE

To provide operational support and assistance for clients and Thrift Store customers in accordance with the Six Points Mission Statement with a positive, professional, compassionate attitude.

 

Supervision

The Residential Program/Facilities Manager (RPM) together with the SLS Program Manager (SLSM) and Day Program Manager (DPM) will directly supervise this position.

 

Program Assistance

As necessary, provides direct hands-on assistance to clients (as assigned) within the following areas:

  • Personal Care: eating/drinking, dressing, bathing, toileting, personal hygiene, emergency response, medications, supervision, health monitoring, and positioning/transferring.
  • Household: money management and disbursements, shopping assistance, laundry, meals assistance, household cleaning and maintenance.
  • Mentoring: decision making, planning, appointment scheduling, safety, health, dietary assessment, parenting and/or relationship education and support.
  • Transportation: arranges for or directly provides transportation services to access community services, training programs, employment, appointments, shopping, banking, events or social activities and accurately tracks and logs all trips.
  • Community Accessibility: Acquisition of community assistance, recreational/community activities, adaptive skills, socialization, and supplies.Operational Support – General Client Services
  • Assists managers in identifying and correcting obstacles and/or hazards associated with client care, health and safety.
  • Provides direct assistance in the completion of objectives for clients as outlined in their Service Plans.
  • Assists with client enrichment programming to help further develop their social and life skills, including, but not limited to, participation in annual Six Points Arts Show, Sugar Plum Festival, Awards Ceremony, Special Olympics, Western State Colorado University activities and other community events as assigned.
  • Documents and evaluates enrichment programming and special events through written assessment and/or photography during the programming/events as assigned.
  • Maintains and/or assists with necessary program documentation as assigned by RPM, SLSM or the Executive Director (ED).Operational Support for Thrift Store/Operations
    • Assistance with (either individually or with clients): processing of donations; display and layout of items for sale; maintenance, cleaning and organization of the store and processing areas; and, adherence to and assistance with safety regulations/laws.
    • Assisting store customers, including: helping with customer inquiries in person and on the phone; checking out/processing customer purchases; general cash register operations and cash processing responsibilities; and monitoring customer – client relations in the store.
  • Assistance with monitoring and/or directing store volunteers and community service volunteers.General Operations

  • Participates in, attends and satisfactorily completes all training, tests and in-service programs, staff meetings, and other designated staff development activities/training to ensure compliance with a total accredited program.
  • Maintains familiarity with agency Policy and Procedures (including updates) pertaining to client programs and services and complies with the requirements of a total accredited program.
  • Assists with cleaning, maintenance and overall appearance of entire facility and grounds and client residences as needed or as assigned by the RPM, SLSM, DPM or ED.
  • With proper training, assists in Thrift Store as approved by the DPM and ED

Other reasonable duties as assigned by the RPM, SLSM, DPM or ED.

Community Relations

Serves as a professional representative of Six Points with clients, client families/friends, the community and Community Options and provides input for the development and presentation of information to the public concerning the organization’s goals/objectives as requested by RPM, SLSM, DPM and/
