Six Points Services

Six Points Evaluation and Training, Inc. provides a full complement of services in Gunnison and Hinsdale Counties of Colorado.

A Broad Range of Services

Six Points currently serves people with intellectual disabilities (see HOME page for eligibility information and a link to the State of Colorado)
and people with Traumatic Brain Injury.

Residential Comprehensive Services

For clients who live in a variety of settings, these all-inclusive services assume total responsibility for the client’s welfare, and include:

  • Mentoring – decision making, planning, appointment scheduling, safety, health, dietary assessment, relationship education and support, and medical assistance)
  • Transportation – accessing community services, training programs, employment, appointments, shopping, banking, civic events or social activities, and
  • Community Accessibility – acquisition of community assistance, recreational/community activities, adaptive skills, socialization, and acquiring supplies.
  • Community Integration – establishing and engaging in connections in the community

Supported Living Services (SLS)

For clients who live in a variety of settings, services in SLS are based on individual client choice and are provided in areas in which assistance is needed.  Services available include:

  • Mentoring – decision making, planning, appointment scheduling, safety, health, dietary assessment, relationship education and support, and medical assistance)
  • Transportation – accessing community services, training programs, employment, appointments, shopping, banking, civic events or social activities, and
  • Community Accessibility – acquisition of community assistance, recreational/community activities, adaptive skills, socialization, and acquiring supplies.
  • Community Integration – establishing and engaging in connections in the community

Vocational/Supported Employment/Day Programming

Providing job services and day programs for clients including:

  • Employment assessment and review
  • Vocational training, specialized habilitation and one-on-one support in conducting job tasks
  • Job placement assistance
  • Serving as the liaison between outside employers and Six Points’ clients
  • Supported community connection

Division of Vocational Rehabilitation (DVR)

As a vendor with the Colorado Division of Vocational Rehabilitation, Six Points may provide employment assistance to any individual (age 15 and older) with a qualifying disability, not limited to intellectual disability, including:

  • Career exploration
  • Job-seeking, and
  • Maintaining successful employment.

Community Thrift Store

The Six Points Community Thrift Store serves the entire Gunnison Valley as the largest thrift store in the region.

The Thrift Store provides an outlet for residents to donate gently used goods instead of throwing them in the trash, thereby contributing to the reuse philosophy that helps sustain our natural resources and environment.

It provides a very important source of affordable merchandise for the low-income population of Gunnison Valley, as well as for several other agencies and at-risk populations that Six Points supports through free donations.

The Thrift Store also serves as a place of employment for numerous clients with disabilities.

Simplified Summary

Six Points offers five kinds of services for people with intellectual disabilities. Residential services take full care of a client’s needs and well-being. Supported living gives help based on what each client wants and chooses. Vocational and day programs provide job support and daily activities for clients. As a vendor with the Colorado Division of Vocational Rehabilitation (DVR), Six Points may give help to anyone age 15 or older with a qualifying disability. The Community Thrift Store gives people a place to donate or buy gently used items and also provides jobs for many clients.
